COMPLETE the survey for the 1st time:

1. In Section 1 below, choose the league name, survey name, and your name in the 3 dropdown menus, and then click "Submit".

2. Refresh this page, fill in the form in Section 2, and then click "Submit / Update Survey".

EDIT your responses after submitting the survey:

To edit your response(s) after submitting the form, choose the field(s) you wish to edit in Section 2, and click "Submit / Update Survey".
